I confirm a patch test has been performed (if required) with no adverse reaction, and the information above is correct to my knowledge. I am not aware of any reasons why the treatment should not be performed. I am aware that even though a patch test has been carried out, reactions can occur/develop at any time and this is not a guarantee that a reaction will never take place. I therefore release my technician/trainer of any responsibility in the event a reaction does occur. I agree to follow the aftercare I have been given.

I am collecting the data on the client consultation forms to ensure that I can safely perform your treatment & to be able to contact you with regards to appointments/marketing. I need to store this data to comply with my public & product liability insurance & to be able to safely continue treatments & keep records of all products used should any issues/allergies/problems arise.

When taking card payments I use a third party Izettle, where only your name & last 4 digits of your card are stored. I collect this data so that I can complete my accounts accordingly for HMRC. This data is not shared with anyone and is on a secure device.

We are now digital on all forms, these will collect the same data as the paper forms & will be stored on the secure Ovatu booking system. My iPad is password protected.

I will store ALL necessary data for 7 years to comply with my legal obligations but it will be reviewed on a yearly basis where you can opt out.
